How to make a referral

Any professional or service provider having contact with the family can make a referral as long as they have the family's consent. Healthy Babies Healthy Children is a voluntary program.

Referrals can be made during pregnancy or anytime up to age 6 years.

Any family with higher than usual needs in terms of medical, socio-economic or psychological challenges is eligible for referral to the Healthy Babies Healthy Children Program. Click here to fill out the Healthy Babies Healthy Children Referral Form

  • Child Care Provider
  • Teachers
  • Social Service Workers
  • Clergy
  • Family or Friends
  • Self Referral

Once a family has been referred to the Healthy Babies Healthy Children Program, an assessment process will determine the need for assistance, ensure follow-up, link the family with appropriate services and determine if home visiting by a Public Health Nurse and Family Home Visitor is required.
