Under the Day Nurseries Act, all children attending a licensed day care or day nursery must be immunized according to the recommended immunization schedule for their age.
The operator of the licensed facility has a legal obligation to make sure that all the children in their care are immunized according to the recommended immunization schedule for their age.
It is the parents' responsibility to inform the Health Unit of their child's immunizations so that records are up to date. Health care providers do not automatically inform the Health Unit when vaccines are given. See below for options on how to update your record.

How do I send any new immunization information to the Health Unit?
If your child gets a new immunization from a health care provider other than the Health Unit, please sent in that information. This will ensure that your child's record is up to date.
To send in new information, you have four options:
Complete the online form - go to the Health Unit's new SECURE website to submit the information - click here
Fill out the Day Care Entry Immunization
form; click here
for a copy.
Use a sheet of paper to list the name of the vaccine, the date given, and the name of the doctor or facility that gave the vaccine. Also, please include the child's name, date of birth, parent(s) name, phone number, address and day care on a separate piece of paper.
Photocopy your child's yellow immunization card. Make sure to include the child's name, date of birth, parent(s) name, phone number, address and day care on a separate piece of paper.
Once you have an up-to-date listing of all vaccines, please forward to the Vaccine Preventable Disease program:
- Fax to 625-4828
Mail to: Vaccine Preventable Disease program, Thunder Bay District Health Unit, 999 Balmoral Street, Thunder Bay, ON P7B 6E7
Exemptions of immunization based on medical, religious or conscientious grounds may be obtained from the Health Unit. If an exemption is required, please call the Health Unit at 625-5925 or 1-888-294-6630, ext. 5925 to make an appointment with the Commissioner of Oaths.
