Under the Day Nurseries Act, all children attending a Licensed Day Care, Licensed Day Nursery or School must be immunized according to the Recommended Immunization Schedule for their age.

It is the parents' responsibility to inform the Health Unit of their child's immunizations. If your child has or develops any allergy, drug sensitivity or significant illness, please notify your family physician and the Health Unit immediately.

To provide immunization information to the Health Unit please:

• Fill out the School Entry Immunization form provided by the daycare or school (click here for a copy) OR
• Simply use a sheet of paper and list the name of the vaccine, the date it was given, and the name of the doctor or facility that gave the vaccine, and fax or mail it to the Health Unit's Vaccine Preventable Diseases Program:

Exemptions of immunization based on medical, religious or conscientious grounds may be obtained from the Health Unit.  If exemption is required please call the Health Unit at 625-5925 to make an appointment with the Commissioner of Oaths.
