An immunization record, or yellow card, keeps track of all immunizations, or vaccinations, that you or your child has received. It is important to make sure that this record is kept up-to-date and stored in a safe place with your other important documents.
Personal immunization records may be required for school, work or travel. An up-to-date immunization record will prevent unnecessary extra shots. |

The Health Unit has a record of all immunizations given by the Health Unit's nursing staff.
It is important to know your health care provider does not automatically inform the Health Unit when you are given a vaccine. It is the responsibility of an individual, or a parent/guardian, to let the Health Unit know of any immunizations received from a health care provider.
