School Immunization

The Health Unit offers school immunization clinics throughout the year.

Before each clinic, public health nurses will assess the immunization record of each student.  If that student is not up to date for his/her age, they will be offered the immunization at their school clinic.

Consents are sent home to parents before the clinic.

Is it my responsibility to make sure the Health Unit has my child’s vaccination history?

Yes, it is parents’ responsibility to inform the Health Unit of their child's immunizations. The Health Unit will have a record of any vaccines administered by Health Unit nurses.  Health care providers do not automatically inform the Health Unit when vaccines are given.

If your child receives a vaccine from another health care provider, please send this information to the Health Unit.

You have two options:

Once you have an up-to-date listing of all vaccines, please forward to the Vaccine Preventable Disease program:

  • Fax to: (807) 625-4828
  • Mail to: Vaccine Preventable Disease Program, Thunder Bay District Health Unit, 999 Balmoral Street Thunder Bay, ON P7B 6E7
