Every student's immunization record is assessed on a yearly basis. If a student is not up to date for his/her age, immunizations will be offered at your childs school. Consents will be distributed prior to this clinic.

PARENTS' RESPONSIBILITY TO PROVIDE VACCINATION HISTORY
It is parents’ responsibility to inform the Health Unit of their child's immunizations. Please supply the name of the vaccine, the date it was given and the name of the doctor who gave the vaccine. The Health Unit only has this information on file if the vaccinations were administered by the Health Unit or if this information had been given to the Health Unit previously.
To have your child’s vaccinations updated, the Thunder Bay District Health Unit offers a walk-in immunization clinic for all individuals over 1 year of age.
SCHOOL REGISTRATION
Under the Immunization of School Pupils act, all children attending school must be immunized according to the Recommended Immunization Schedule for their age. To have your child’s immunization assessed for school requirements, please print and complete the following form and return to the Health Unit

If a parent chooses not to immunize their child, exemptions to immunization based on medical, religious or conscientious grounds may be obtained from the Health Unit by calling (807) 625-5971 or (807) 625-8346.
VACCINATIONS OFFERED AT SCHOOL
Boosters
- DPTP (diphtheria, pertussis, tetanus, polio) and MMR (measles, mumps, rubella) are offered to students in Senior Kindergarten.
- dTap (diphtheria, tetanus, acellular pertussis [whooping cough])
14-16 year old boosters is offered to students at the secondary school level.
Hepatitis B (Hep b)
- Hepatitis B is provided to all grade seven students.
- The schedule for Hepatitis B series consists of two doses. The second dose follows four to six months after the first.

Meningococcal C-Conjugate (Men C)

Children born on or after September 1, 2003 will be eligible to receive a single dose of the meningococcal C conjugate vaccine on or shortly after their first birthday.

In grade 7, students who have not yet received the Men C-Conjugate vaccine will be eligible to receive it.
- All youth 15-19 years of age will also be offered the vaccine.

Human Papilloma Virus (HPV)
- Grade 8 girls will be offered the HPV (Human Papilloma Virus) vaccine free of charge from the Health Unit upon receiving parental consent
- 3 doses are required for full protection
