What the codes mean:

DTaP-IPV = Diphtheria, Tetanus and Acellular Pertussis (whooping cough) / Inactivated Poliovirus vaccine

DTaP-IPV:

All children may not have received 4 doses as the number of doses of this combination- it depends on the age at which they started receiving this vaccine.

Hib = Haemophilus influenza (meningitis)

PneuC 7 = Pneumococcal conjugate

MMR = Measles, Mumps, Rubella (german measles)

MenC-C = Meningoccal C conjugate

VZ = Varicella (chickenpox)

HEP B = Hepatitis B series (NOT mandatory)

Td = Tetanus and Diphtheria
dTap = Diphtheria, Tetanus & Acellular Pertussis (whooping cough)

Flu = Influenza
