Immunization of School Pupils Act 1982

According to the Immunization of School Pupils Act, students must be adequately immunized against the following diseases:

  • diphtheria
  • tetanus
  • polio
  • measles
  • mumps
  • rubella

Every students immunization record is assessed on a yearly basis. If a student is not up to date for his/her age, immunization will be offered during the school clinic. Consents will be distributed prior to the date of the clinic. Click here for a schedule of what vaccines are required at which age.

Parent's Responsibility

If a parent chooses to have their child immunized through their family physician, it is the parents responsibility to inform the health unit of their child’s immunization.

Please supply the health unit with the name of the vaccine, the date it was given and the doctor’s name who gave the vaccine. Remember to ask your physician to update your child’s yellow immunization card.

Keep your immunization card in a safe place! This record may be required by future employers, educational facilities and for travel.

Before your children start school, send the health unit the information. Below are some forms to print off, fill out and bring to the health unit if you have misplaced the forms in the registration package you received from school
