The Ontario Ministry of Health provides publicly funded Hepatitis A  and Hepatitis B  vaccine for eligible individuals.  Physicians can order the publicly funded vaccines by completing the order form and fax it to (807) 625- 4828 Ordering of Vaccines
You will be required to pick up your vaccine order at the Thunder Bay District Health Unit.

Please note that the vaccine is only available for the high-risk groups as listed on the order form. If you have any questions regarding the availability of the vaccine please contact Connie Lake at (807)625 5942.

Vaccine is also provided at the Thunder Bay District Health Unit's weekly immunization clinic every Wednesday at 999 Balmoral Street from 1:30 - 6:00 PM.
