Library Loan Request Form
Please fill out the form below if would like to borrow resources from the
library. All fields are required, incomplete or inaccurate requests cannot be
processed. The loan period is one week. It is the
responsibility of the borrower to ensure that all materials are returned to the
Thunder Bay Distrcit Health Unit Resource Centre by the due date. If any
materials are misplaced or lost, the cost of replacement will be the
responsiblity of the borrower.
