- Records needed: TBDHU needs a copy of the vaccine records of all students entering school including those:
- starting Junior Kindergarten (JK)
- previously homeschooled
- who have moved from a home community
- new to Ontario; moved from another province or country
- Can still attend school: Students can still attend school as TBDHU waits to receive their vaccine records. However, if a student's records have not been received by the start of the suspension process for their school, they could be suspended. Why are records needed?
- Why? TBDHU staff need an accurate history of your child’s vaccines so they can determine whether your child has all the recommended vaccines including those required under the Immunization of School Pupils Act.
- Health care providers don't regularly submit vaccine information: Most health care providers do not forward vaccine information to TBDHU. So if your child received a vaccine at a health care provider’s office, it is your responsibility as their parent/guardian to share this information with the TBDHU. All vaccines given by TBDHU staff, or any other Ontario public health unit, will automatically be added to a student's vaccine record.
How can records be submitted to TBDHU?
- Options available: Vaccine information can be shared with TBDHU in a variety of ways that are listed at TBDHU.com/vaxreport.
- Send a photo: One of the easiest options is to upload a photo of any vaccine information and send through a safe and secure link.
- Register for an ICON account: TBDHU recommends parents/guardians also register for an Immunization Connect Ontario (ICON) account. An ICON account will allow parents/guardians to access their child's vaccine records quickly and easily in the future if they are needed for medical, travel, employment or post-secondary education. The postal code used to register must match the postal code in the vaccine record. Families experiencing challenges accessing ICON can call 807-625-5908 or toll-free 1-888-294-6630 (ext. 5908).
