Under the Child Care and Early Years Act, all children attending a licensed child care centre (daycare) must be immunized as required by the local Medical Officer of Health (MOH). The operator of the licensed facility has a legal obligation to make sure that all the children in their care are immunized according to these requirements.
Parents/guardians will be asked to complete the Licensed Child Care Entry - Immunization (IMT-402) form. Note: for the most recent version, please call (807) 625-5900 and ask to speak to the Vaccine Preventable Diseases team. The most recent version will be added here soon.
If the record is not complete according to the Publicly Funded Immunization Schedule of Ontario, parents/guardians will receive communication from the Health Unit outlining their options.
There may be medical or conscience/religious reasons for not immunizing children. If this is the case, the parent/guardian must submit approved Ministry of Education forms.
As of August 29, 2016, one of the following forms must be used:
- A Ministry of Education Statement of Medical Exemption form. The form must contain the health care providers (physician or nurse practitioner) signature and license/registration number.
