Immunization records can be reported to the Health Unit via the following options:
- Use Immunization Connect Ontario (ICON) - Online system for reporting and obtaining immunization records. Learn more here.
- Have the immunization record faxed to 1-807-625-4828; you can do it yourself or have your health care provider fax it for you
- Mail it to the TBDHU: 999 Balmoral Street, Thunder Bay, ON P7B 6E7
The Immunization of School Pupils Act requires that all students have proof of vaccination against several different infections. These infections are covered under Ontario's Immunization Schedule.
Because this act requires the Health Unit to keep an updated immunization record (or valid exemption) for every child attending school in the district, children starting school must submit the School Entry Immunization Form (PDF) to the Health Unit. This includes children new to school (e.g. junior kindergarten) as well as children moving to the Thunder Bay District from another jurisdiction.
Under the Child Care and Early Years Act, all children attending a licensed child care centre (daycare) must be immunized as required by the local Medical Officer of Health (MOH). The operator of the licensed facility has a legal obligation to make sure that all the children in their care are immunized according to these requirements.
Parents/guardians will be asked to complete the Licensed Child Care Entry – Immunization Form (IMT-402) (PDF).
If the record is not complete according to the Publicly Funded Immunization Schedule of Ontario, parents/guardians will receive communication from the Health Unit outlining their options.
Adults can also choose to report their own records to the Health Unit, but are not required to do so.
