Booking a Vaccine Appointment at TBDHU

Book an appointment using the online booking system for:
- Routine vaccines according to Ontario's FREE publicly funded vaccine schedule
- RSV vaccine for those who are eligible
- Pneumococcal vaccine for those who are eligible

Respiratory Season Vaccines
- Find more information on each of the fall vaccines (COVID-19, flu and RSV) by visiting TBDHU.com/fallvax2025.
- COVID-19 and Flu vaccines: Available through pharmacies and health care providers.
- RSV: Only available through a health care provider, NOT available at pharmacies.

Submitting Vaccine Information BEFORE your appointment
Thunder Bay clinic
- Submit vaccine information about any vaccines that were received from another health care provider at least 1 week before your appointment by using one of the options listed on our vaccine reporting web page.
- If we do not have up to date vaccine information for you or your child you may be asked to reschedule your appointment.

APPOINTMENTS AVAILABLE FOR:
- Publicly funded routine vaccines for adults 18 and older without a health care provider
- Publicly funded routine school-aged vaccines for children 4 to 18 years of age. The vaccines that they will be offered will depend on age and other eligibility criteria.
- Publicly funded vaccines under Ontario’s high risk vaccine programs
- Some vaccines are available for purchase for those who are not eligible for publicly-funded vaccines
- The Imvamune® vaccine for protection against MPOX
HOW:
- Book a vaccine appointment at the Main Office Clinic in Thunder Bay by clicking HERE.
- Submit vaccine information about any vaccines that were received from another health care provider at least 1 week before your appointment by using one of the options listed on our vaccine reporting webpage.
- Translate any international vaccine documents and submit them at least one week before an appointment. Appointments may need to be rescheduled if the documents are not translated. Please contact the Thunder Bay Multicultural Association for assistance.

Lakehead Healthy Start Program
- Lakehead Nurse Practitioner-Led Clinic is now accepting infants and children up to 5 years of age without a health care provider.
- No referral is needed.
- Please call their office to make an appointment for a well-baby visit and any immunizations/vaccines; 807-475-9595.
First Five Program
- Norwest Community Health Centres in Thunder Bay is accepting infants and children up to 5 years of age without a health care provider.
- No referral is needed
- Call 807-622-8235
TBDHU does not provide well-baby health assessments for children 2-18 months of age.
- Health assessments are routinely paired with immunization appointments.
